Senior Care Coordination
When long-term, intensive case management is needed, this program provides senior advocacy-based care management by experienced, credentialed staff. The mission of this program is to provide an individualized care plan which seeks to reflect and preserve the senior's values and lifestyle choices while maximizing opportunities for positive change and maintaining quality of life.
This is a fee-for-service program.

Information & Assistance
An Information & Assistance Specialist is available to answer questions and provide referrals by telephone. If the caller would prefer face-to-face contact, a drop-in visit may be scheduled or, if the senior is unable to come to us, a Case Worker can be sent to the home for further case management services.

Case Management
Case Workers are available to travel to the home of the senior for a confidential assessment of needs. Along with the senior, a care plan is created and the Case Worker can assist with appropriate referrals to other resources in the county or programs within the Council On Aging.

Senior Well Check
If a senior is in need of a regular check-in visit because family cannot be there, the Senior Well Check Program might be the answer. An experienced Case Worker can serve as a gateway to other senior resources and will provide check-in visits to monitor the health, safety, social, nutritional, medical and daily living needs of the senior.
